Print the request services form to fill out and provide at pickup time. If you do not have a printer or prefer to save paper, click the link below to download an app you can use to fill out the form electronically. The app is available on Android or iOS for iPhone or iPad. You can then email the electronically completed form.
When you have your labeled blankets and other items and your request form ready, contact us to arrange pick-up or delivery.
